During the travel usually we get tired by journey. Because of changes climate, place and adjust with new type of food, water, air of new places make some trouble in the body. Sometime we get sick also. So during traveling-
# Take adjustable cloths for the new places weather,
# Take new type of food small amount, don’t eat those food which is not testable to you.
# Take first aid with you.
# Keep common medicine with you which you take before.
# Take necessary rest and sleep to get relaxed.

That is a lot of good information. I would recommend that you not only keep your medicine with you, but a list of your Drs and with your medical conditions. You need to make sure that if something happens to you that they know what to treat you for.

First Aid is a MUST-HAVE when you travel. Especially if you know that you can get sick easily. You should have first aid in your bag.
Aside from those suggestions you gave, I think you should consult your doctor before traveling. Check your condition if you are healthy enough to travel around. Research on the weather of your destination. From hot to cold weather or vice versa can really lead you to certain sicknesses. With that, bring clothes that are suitable for the weather.
Remember, no matter how beautiful your destination is but your health is poor and weak, you wouldn't be able to enjoy the trip completely.

I think one of the most important things to carry is a mobile phone so you can make emergency calls if you ever have to, there are lots of tips about travel health here, also carrying spare batteries is useful too.

First aid kit, take advice if necessary on contents.
If you are traveling in countries where toilets do not have washing facilities always use safe hands.
Take fresh wipes and a toilet roll with string thru it to hang round your neck.
Take Imodium tablets with you, in India, Nepal and Africa it is very easy to get very bad stomach problems even when you have been careful where you eat.
Drink plenty of bottled water.
If you take medication, for future reference take a letter written by your doctor saying why you are taking the medication.
When flying keep enough medication in hand luggage just in case of delays.
Don't assume you can purchase the same health/medical products in other countries, if there is something you need take a supply with you.
Take a spectacle prescription with you.
If you have a long flight ahead of you always book accommodation in advance for when you arrive, you can then get straight there and get your head down.
Jet lag can last a number of hours don't fight it get to sleep at the earliest opportunity afterwards you will feel as good as new.
Get up and walk around if you are on a long flight.
Don't drink too much alcohol specially when flying.
Use earplugs, other peoples continuous talk and noise can make you irritated and stressed.
Use sunscreen or cover up in hot climates.
If you have allergies, have the details written on a fob attached to a chain round your neck.
